MBA's primary affiliation is with our insurance carriers, Metropolitan Life Insurance Company (MetLife) for life insurance and The Hartford Life Insurance Company and Hartford Life and Accident Insurance Company for our TRICARE Supplemental Insurance Plans.
To provide MBA members with additional benefits, MBA partners with widely respected companies such as Liberty Mutual, U.S. Bank, Armed Forces Insurance, and LifeLine Screening—these affiliations directly fund MBA’s scholarship for MBA family members.
Simply review the plans on our website and complete the appropriate application. You can apply online or you may sign and date a completed paper application and scan and secure email (call us for instructions), mail, or fax it to MBA.
Additional medical information may be requested by our underwriter. You must be under age 70 when the change or increase becomes effective.
Currently you may make a payment over the phone using a major credit card or debit card Monday through Friday, 8 a.m. to 8 p.m. EST at 1-800-336-0100, option 4. You can also make a one-time payment online using your checking account or major credit card /debit card.
You have the option to list as many people to receive the death benefit as you see fit. You will just need to list them on the Beneficiary Designation form and return it to us for recording. If your beneficiary designation does not fit on the form provided please use an additional form and complete your list of beneficiaries on that form. Attach the additional form to the first form, indicating clearly on each form the number of additional forms attached. For example, if three forms are used, number the forms as follows: 1of 3, 2of 3 and 3of 3. You may access these forms on our website and return them to us ensuring that you have signed and currently dated the forms.
(Note: while you have the option to list as many beneficiaries as you wish, naming more than one may slow down the processing phase of the claim.)
MBA takes pride in fully serving our Service Members and their families; there is no aviation or war clause. This means the full death benefit would be payable if the death were caused by an act of war or military aircraft accident.
A change in duty status does not affect your coverage as long as consecutive payments are received for the coverage. Please ensure that your premium will continue to be paid even though your military allotment might have stopped.
MBA offers Group Term life insurance coverage; therefore, there is no cash or loan value to any of our plans.
Members and dependents have a conversion privilege to an individual policy of life insurance with MetLife, as explained in the certificate of insurance. If you have any questions regarding this privilege, please contact the office of the MBA at 1-800-336-0100 ext 313.
